Embed a Convertly Form
Copy the generated embed
Section titled “Copy the generated embed”- Open Forms.
- Open the active form.
- Choose Embed.
- Copy the complete generated snippet without changing its form token.
- Paste it into the page where the form should appear.
The form token identifies one public form. It is not a reusable workspace API key.
Place the embed correctly
Section titled “Place the embed correctly”Add the snippet through a trusted HTML or code block supported by your website platform. Avoid placing the same form embed multiple times on one page unless you have tested that layout.
The current supported production path is an inline embed. Hosted public form pages are not available yet.
Campaign and consent context
Section titled “Campaign and consent context”The generated embed can collect available URL and page context, including:
- Google and Meta click identifiers
- UTM source, medium, campaign, term, and content
- Landing page and referrer
- Supported ad metadata
- Consent status and timestamp
Do not add tracking parameters manually unless the advertising or analytics plan requires them. Keep consent wording consistent with your privacy policy.
Domain controls
Section titled “Domain controls”When allowed domains are configured, include every legitimate production hostname that serves the form. Do not add wildcard or unrelated domains for convenience.
Verify the live page
Section titled “Verify the live page”- Open the page using a test campaign URL.
- Submit unique test contact details.
- Confirm the success state appears.
- Open the created lead.
- As an Admin or Manager, review available capture and advertising context.
- Confirm the right source, stage, categories, owner, value, and alert recipients were applied.
- Repeat with the same email or phone to verify duplicate behavior.
Troubleshooting
Section titled “Troubleshooting”If the form does not load, check that it is active, the entire snippet is present, the domain is allowed, and the page permits scripts. Use Forms logs to investigate accepted, skipped, duplicate, or failed submissions.
